Log into your account on the Healthy Hearts! Healthy You! Portal and take the following steps to earn 500 points!

Know Your Numbers – complete your assessment and screening

Fill out the 15-minute Health Assessment Questionnaire and complete your Biometric and Blood Screening to help you learn your personal health score. You must complete this questionnaire each program year to qualify for the Reward.

Choose from one of three ways to get your Biometric and Blood Screening:

  1. Schedule an appointment at a Quest Patient Service Center. Find the Patient Service Center closest to you:
  2. Download the Physician Results Form through the Quest portal for you or your spouse’s doctor to complete; or
  3. Attend an onsite Biometric and Blood Screening event at ZOLL.

Quality of Sleep Can Be Life Changing


 

Don’t miss out on a free sleep apnea assessment! Studies link sleep apnea to health issues like diabetes, stroke, heart attack, and arrhythmias. It can also impact work performance and increase the risk of accidents. Find out if you might be at risk for sleep apnea and may be eligible for a FREE WatchPAT® ONE Home Sleep Apnea Test (HSAT).

Sleep apnea is a common sleep-related breathing disorder impacting millions of people every day. It causes an individual to pause their breathing or create shallow breathing while they sleep. “Poor-quality” sleep can have negative effects on many different aspects of our health and well-being. That’s why as part of this year’s Healthy Hearts! Healthy You! program, we are encouraging all employees and their spouses to find out if they may have sleep apnea.

HOW IT WORKS
To initiate the process and earn points toward your reward, follow these steps after registering on the Healthy Hearts! Healthy You! Portal:

  1. Complete your Health Assessment Questionnaire (HAQ). In the Health Behaviors section of the questionnaire, one of the subcategories is called Sleep and Fatigue. Here you will see a question asking if restless nights are affecting your quality of life. This is where you will have the option to select “Yes” to access a free risk assessment for sleep apnea.
  2. Be on the lookout for an email and text message. After selecting “Yes” in the HAQ, you will receive an email introduction from benefits.program@zoll.com. This email will inform you to expect a text message from a SleepMedRx Physician to initiate the screening process. Follow the instructions in the text message to see if you clinically qualify for the WatchPAT® ONE Home Sleep Apnea Test.
  3. Receive your free Home Sleep Apnea Test in the mail. If it is determined that you qualify for the WatchPAT® ONE device, it will be mailed directly to your home address. You’ll also receive another text message with information and specific instructions for using the device.
  4. Access your results and share them with your doctor. Your Sleep Study Report will be emailed to you within one week of completing the test. If your findings suggest that you could benefit from treatment therapy, be sure to discuss your options with your primary care physician or sleep medicine specialist.

Complete a Home Sleep Apnea Test!

  • You can earn up to 150 points toward the 500 points needed to receive your $500 Healthy Hearts! Healthy You! Reward.
    • There are two ways to earn points – a WatchPAT® ONE is sent to you, and you complete the testing process, or during your evaluation it is determined a test is not indicated.

    If you were sent an HSAT during last year’s program but did not complete the test, you can do so this year without electing “Yes” in the Health Assessment Questionnaire to access a free risk assessment for sleep apnea. If you misplaced your PIN, please contact benefits.program@zoll.com.

You deserve to feel great and enjoy life. With the Daily Habits Plans, you will learn to make incremental lifestyle changes, set goals, and practice habits that contribute to lasting health benefits.

HOW IT WORKS
Daily Habits uses behavioral science to help improve your well-being. It is fun, easy to use and science based. Stay motivated, achieve results and be confident that your health is heading in the right direction!

To successfully complete the Plans, you will be expected to register on the Healthy Hearts! Healthy You! Portal, then:

  1. Complete an intake questionnaire prior to starting the Plan. This will help us to get to know you and understand your current situation in order to personalize the activities you will be interacting with as part of the Plan.
  2. Review and complete all the activities that are assigned to you based on your questionnaire.
  3. Complete the outtake questionnaire.

Your progress indicator will show green, yellow, and red. If you see yellow or red, check on what you can do to move back to green in order to meet your goal.

Asthma Plan Breathing is important. Let us help you with managing your asthma and keep symptoms at bay! Although there is no real cure for asthma, it is something you can take control of. By better managing the condition, you can help prevent symptoms such as coughing, wheezing or chest tightness.
Back Health Plan No matter what you do for a living – whether you’re standing, sitting or performing physical activities majority of the time – achieving and maintaining good back health is as important as a well-balanced diet to maintain a good quality of life. Having a strong back foundation can help minimize overuse injury, repetitive motion issues, and keep discomfort within reasonable limits.
Balance Your Diet Plan What you eat matters. Regularly making healthy food choices can help you maintain a healthy weight, reduce your risk for chronic disease and more. With Balance Your Diet, you’ll set a plan to take part in healthy activities that can help you enjoy the many benefits of healthy eating.
Coronary Artery Disease (CAD) Plan This goal is designed to help you manage your CAD by focusing on several daily activities, as well as activities done less frequently that still have a critical impact on your health.
Chronic Obstructive Pulmonary Disease (COPD) Plan This goal is designed to help you manage your COPD by focusing on several daily activities, as well as activities done less frequently that still have a critical impact on your health.
Cope with the Blues Plan Depression has an effect on more than your state of mind. It can also contribute to a wide range of physical problems that can affect everything from your heart to your immune system. The Cope with the Blues Daily Habit Plan is designed to help you overcome depression and help you focus on the positive so you can begin to enjoy life.
Diabetes Plan This goal is designed to help you manage your Diabetes by focusing on several daily activities, as well as activities done less frequently that still have a critical impact on your health.
Enjoy Exercise Plan Staying active is one of the best things for your health. With the Enjoy Exercise Plan, you’ll set a goal to take part in healthy activities that interest you and stick with them for at least 28 days.
Heart Failure Plan This goal is designed to help you manage your Heart Failure by focusing on several daily activities, as well as activities done less frequently that still have a critical impact on your health.
Hypertension Plan This goal is designed to help you manage your high blood pressure by focusing on several daily activities, as well as activities done less frequently that still have a critical impact on your health.
Keep Stress in Check Plan Keeping stress in check is important for your well-being. Over time, too much stress can help contribute to a range of health problems — from headaches to obesity, diabetes and heart disease.
Lose Weight Plan Losing a little weight could have a big impact on your health. Dropping just five to ten percent of your total body weight can help improve your blood pressure, blood cholesterol and blood sugars.
Maternal Health – Trimester 1 Plan Having a healthy pregnancy is one of the best ways to promote a healthy birth. Getting early and regular prenatal care improves the chances of a healthy pregnancy. It’s never too early to connect with your health care provider.

When someone has a healthy balance, they report less stress and increased satisfaction with life. How to achieve work-life balance is different for each person, though. No two people are in the same situation or have the same strengths. So, what works for one person might not work for you.
 
With the Work-Life Balance Plan, we will help you become more aware of your habits that may be undermining your ability to stay within your boundaries as well as encourage activities to help you achieve a more satisfying work-life balance.

Get Your Reward

It pays to get healthy!
When you (and/or your spouse) complete the steps and earn 500 points by the Reward deadline, you qualify for rewards. Your spouse can earn rewards even if you do not participate.

Reward Payout
Rewards earned in the current plan year will be paid out early the following year.
(Example: If you earned your reward in 2024, you received your reward in early 2025.) For those who earned their 2025 Rewards, medical premium discounts have been applied, and cash payments will be disbursed in February, 2025.
