When your employment ends at ZOLL, you have some decisions to make about your benefits. Be sure to read the information you receive carefully, so you can make the best choices for you and your family. No matter where you go, we wish you luck on your next endeavor.

Medical, Dental and Vision Benefits

For all employees, your medical, dental and vision coverage will end on the last day of the month in which your employment ends as well for any dependents that age out of coverage.

You will then have the option of continuing your current medical, dental and vision plans through COBRA for up to 18 months.

  • The cost is 102% of the monthly premium, which includes the full premium cost along with an administrative fee.
  • You must elect continuation of coverage within 60 days of your end of coverage or when you receive notification.
Life and AD&D

Life and AD&D Insurance benefits end on your last day of work.

  • You can convert your benefits to an individual policy at a group rate.
  • A conversion package will be sent to you and must be completed and returned within 31 days from the date benefits ended.
Disability

Short-Term Disability and Long-Term Disability coverage ends on your last day worked.

Flexible Spending Accounts (FSA)

Your FSA contributions continue through the end of your last pay period.

  • You can submit claims through March 31 of the following year.
  • All services must be rendered before the date of your last day worked.
Pet Insurance

You can convert your plan to an individual policy through Nationwide. Keep in mind that when you renew your policy, you will lose your group discount.

Time Away from Work

Accrued, unused vacation time prior to your last day worked will be paid out according to the accrual schedule. You will not receive payment for unused holiday or sick time.

401(k) Plan

Contact Fidelity Investments to find out your options with your account.

  • If you have an outstanding loan balance, you must pay it back in full within 60 days of your last day of work.
Payroll, Pay Statements and Tax Form

You can access your existing pay statements and W-2 forms for up to three years from the date your last statement was issued.
