Special Needs Support

ZOLL offers support to parents raising children with medical, behavioral, and learning conditions. Provided with your medical, dental and vision plans are tools to help you:

  • Navigate care and find effective therapies
  • Resolve coverage questions
  • Connect to community resources
  • Create a care plan for your child

Care.com also offers free Care Talks, exclusive events from expert speakers offering advice and guidance to help you better care for yourself and your family, including those with special needs and elder care. Visit their events page to see upcoming events throughout the year.

You may need to verify your disabled dependents over the age of 26 in order to continue coverage. Those processes are outlined below.

Aetna
  • Employee must submit these forms:
  • Completed forms can be faxed or mailed to the address/fax information included in the form.
  • Once approved by Aetna, the employee will receive notification if this disability needs to be re-certified each year, or if it is deemed permanent.
UnitedHealthcare
  • Employee must submit UHC Statement of Eligibility Application.
    • Page two of the application must be completed by the attending physician.
  • Completed application can be faxed or e-mailed to the fax/e-mail information included in the application.
  • Employee must submit application to UHC within two months (and no sooner than six months) of losing dependent status.
  • Once approved by UHC, the employee will receive notification if this disability needs to be re-certified each year, or if it is deemed permanent.
Delta Dental
  • Employee must submit Delta Dental Disabled Dependent Application.
    • Final section of the form must be completed by the attending physician.
  • Completed form should be mailed to the address included in the form.
  • Once approved by Delta Dental, the employee will receive notification if this disability needs to be re-certified each year, or if it is deemed permanent.
VSP

No certification is necessary. Workday should indicate that the dependent is disabled.

Workday Enrollment

It’s important to know how to mark a dependent as disabled in Workday:

If you are updating an existing dependent Contact your local HR Benefit Advocate
If you are adding a new dependent When adding a dependent to your plan, enter their personal information and check disability box
