Follow this Checklist for Next Steps:
Contact Human Resources within 31 days of your marital status change to add your spouse to benefits.

- You will need to provide proof of the event, such as a copy of your marriage license.
Evaluate your plan contributions to determine whether your paycheck deductions will change.
Consider adjusting your contributions to your Flexible Spending Account or Health Savings Account.
- Visit NetBenefits.com to review your current contributions.
Update your 401(k) beneficiaries at NetBenefits.com.
- Under federal law (ERISA), if you are married, your spouse is automatically your primary 401(k) beneficiary. You may only designate someone else if your spouse provides written, notarized consent. For questions, call Fidelity at 1-800-835-5098.
Review your Life and AD&D beneficiaries in Workday for both Basic Life and Voluntary Life Insurance.
Consider enrolling your spouse in Voluntary Life and AD&D Insurance. Please note, you must have Voluntary Life and AD&D coverage for yourself before you can add your spouse to Voluntary Spouse Life Insurance.
