Basic Life and AD&D

ZOLL automatically provides basic life and AD&D insurance coverage through New York Life in the amount of 2x your base salary (plus commission). The best part – ZOLL covers the full cost!

Basic Life and AD&D
Eligibility Active Full Time employee working 30+ hours per week
Cost Coverage is paid 100% by ZOLL
Benefit Amount Twice your annual compensation, rounded to the next $1,000, subject to a maximum of $500,000

  • Once you reach age 70, the benefit amount is reduced to 35%

Choose a Beneficiary

Don’t forget that you must have a primary beneficiary (18+) entered into Workday so that your money goes where you want it to in the event of your death.

Employee Voluntary Life & AD&D
Eligibility Active Full Time employee working 30+ hours per week
Cost Coverage is paid 100% by the employee
Benefit Amount Choose from a minimum of $10,000 to a maximum of $500,000 in $10,000 increments *subject to Guaranteed Issue

  • Once you reach age 70, the benefit amount is reduced to 35%
Guaranteed Issue
  • During the initial eligibility period is the lessor of 3x your annual compensation or $250,000
  • Any amount elected in addition is subject to the evidence of insurability process through Reliance Standard
Spouse Voluntary Life
Eligibility Employee must elect Voluntary Life to elect dependent spouse coverage. Spouse amount cannot exceed 50% of the employee’s elected amount

  • Spouse must not be legally separated or divorced from you
  • A person cannot have coverage as both an employee and a dependent
    For example: If your spouse also works at ZOLL, they cannot purchase employee Life and also be covered as your dependent for Spouse Voluntary Life
Cost Coverage is paid 100% by the employee
Benefit Amount Choose from a minimum of $25,000 to a maximum of $250,000 in $25,000 increments *subject to Guaranteed Issue

  • Once your spouse reaches age 70, the benefit amount is reduced to 35%
Guaranteed Issue
  • During the initial eligibility period is $25,000
  • Any amount elected in addition is subject to the evidence of insurability process through Reliance Standard
Child(ren) Voluntary Life
Eligibility Unmarried, financially dependent children up to 26 years of age, including Natural, adopted, step, foster and disabled children

  • Only one insured spouse may cover dependent children
    For example: If your spouse also works at ZOLL, they cannot purchase Child Life Insurance for the same dependents as you. Your children cannot be covered twice
  • Employee must elect Voluntary Life to elect Child Voluntary Life Insurance
Cost Coverage is paid 100% by the employee
Benefit Amount $10,000 per eligible dependent child
Guaranteed Issue Evidence of Insurability is never required for dependent child coverage
Submitting Evidence of Insurability

Any time you elect Voluntary Life Insurance above the listed Guaranteed Issue, Evidence of Insurability (EOI) is required for approval of the benefit amount. The preferred method for completing EOI is online.

  • When you elect Voluntary Life Insurance amounts above the Guaranteed Issue during your enrollment process in Workday, you will be sent an additional task. It will notify you to complete the evidence of insurability process:
  • New York Life will contact you after your submission of the EOI form
  • Once a decision is made by New York Life, you will receive an email from Workday that your excess amounts have been approved. Until approved, you will automatically default to your Guaranteed Issue amount
