EMPLOYEE ELIGIBILITY

You are eligible for benefits if you are a full-time employee working a regularly scheduled work week of at least 30 hours.

DEPENDENT ELIGIBILITY

You can enroll your qualified dependents for medical, dental, vision, and life insurance benefits. Eligible dependents include:

  • Legal spouse
  • Natural, step, legally adopted, or eligible foster children through the last day of the month in which they turn 26
  • Children recognized under a Qualified Medical Child Support Order

WHEN TO ENROLL

ZOLL gives you three opportunities to enroll in benefits during the year:

1. Open Enrollment
Open Enrollment is your once-a-year opportunity to enroll in or make changes to benefits for the next plan year. Your choices will become effective on January 1 of the following year.

2. New Employees
Newly hired employees have 31 days from their date of hire to enroll in coverage. If you miss your enrollment period, you will not be able to enroll until the next annual Open Enrollment period, unless you have a qualifying life event during the year.

3. Qualifying Life Events
If you have a qualifying life event during the year, you have 31 days from the date of the event to make changes to your benefits. Examples include:

  • Marriage, divorce, adoption, or death
  • Employment change for you or your spouse
  • Loss or gain of alternative insurance coverage
  • Dependent becomes ineligible (for example, your child ages out of coverage because they are 26 years old)

ENROLLING IN WORKDAY

New enrollments and changes to existing elections are made in Workday.

Open Enrollment and New Employees
During Open Enrollment or if you are new to ZOLL, you will see a task in your Workday inbox to complete your enrollment.

Qualifying Life Event
If you have experienced a qualifying life event and need to make changes to your current benefit elections, click into your benefits app from your Workday homepage and select “change benefits”.

ZOLL Opt-Out Cash Incentive
If you waive enrollment in ZOLL’s medical, dental, and vision plans and have access to coverage under another plan (such as through Medicare, your spouse’s plan, a healthcare exchange or Tricare), ZOLL will pay you an incentive of $100 per month.

To receive incentive, you must elect the plan “Opt Out Med/Den/Vis” in Workday during the benefit enrollment process. By selecting this option, you are choosing to waive coverage and provide proof of alternative medical coverage. After submitting the enrollment task, you will receive a follow-up task in your Workday inbox that will prompt you to upload a picture of your alternate existing medical coverage.
