Any time you elect Voluntary Life Insurance above the listed Guaranteed Issue, Evidence of Insurability (EOI) is required for approval of the benefit amount. The preferred method for completing EOI is online.
- When you elect Voluntary Life Insurance amounts above the Guaranteed Issue during your enrollment process in Workday, you will be sent an additional task. It will notify you to complete the evidence of insurability process:
- New York Life will contact you after your submission of the EOI form
- Once a decision is made by New York Life, you will receive an email from Workday that your excess amounts have been approved. Until approved, you will automatically default to your Guaranteed Issue amount
